We know that our patients' time is valuable, and we keep our patients informed at every phase of the treatment process in order to make the best possible use of their time. Before visiting the office of Head and Neck Surgical Associates in Portland, Oregon for dental surgery, orthognathic surgery, or another of the many procedures we offer, we encourage our patients to review the following information to learn more about our policies governing appointments, financial and insurance matters, and the laws that protect your privacy.
In order to better evaluate your needs, we recommend an initial consultation. At this appointment one of our physicians will review your medical history and discuss the appropriate surgical options. We offer a wide variety of surgical and non-surgical treatments, including dental surgery, orthognathic surgery, surgical oncology, and many different cosmetic procedures. You will have a chance to ask questions and better prepare yourself for the procedure(s), if applicable. In some circumstances, the consultation and procedure may be performed at the same appointment. If you have an urgent problem, we'll accommodate you as soon as possible.
Our office hours are 8:00 am - 5:00 pm, Monday through Friday. We are closed for lunch from 12:15 pm - 1:30 pm, as well as on Saturday and Sunday. You may call (503) 224-1371 and select Option 1 to schedule an appointment. Please remember, if you have an emergency, dial 911 or visit the closest hospital ER (emergency room).
Our physicians are often called upon to render emergency treatment at one of Portland’s local hospitals, which may produce a delay in the appointment schedule for patients scheduled for non-emergency dental, orthognathic, or other surgery. We will make every effort possible to contact you should this occur. Therefore, please equip us with all of your appropriate contact information.
Be assured that if you need emergency treatment, you will be seen as quickly as possible.
At Head and Neck Surgical Associates in Portland, Oregon, specializing in dental, orthognathic, and oncological surgery, as well as other surgical treatments of the head and neck, we make every effort to provide you with the finest care and the most convenient financial options. Fees are established by diagnosis, treatment and follow-up of your condition. Fees also reflect the complexity, degree of difficulty, and risk involved with the procedure. Should you require surgery, you may incur additional fees from the hospital, anesthesiologist, radiologist, and other healthcare providers. These fees will be billed separately and are not associated with Head and Neck Surgical Associates.
We accept cash, money orders, personal checks (with valid photo ID), VISA®, MasterCard®, and Discover® credit cards. Fees are due when services are rendered.
Our practice can only serve our patients well if its financial policy is clearly stated. Please print a copy of our financial/credit policy for your reference, and read it carefully. Employers and other plan sponsors offer dental and medical insurance benefits for a variety of reasons, including promotion of general health, and attraction and retention of high-quality employees. Treatment decisions must be made by you and your surgeon. While dental and medical benefit coverage should be taken into account, it should not be the deciding factor in your choice of treatment.
Your insurance policy is an agreement between you and your insurance company. We work hand-in-hand with you to maximize your insurance reimbursement for covered procedures. Insurance companies do not set our fees, but they do establish a list of fees that they refer to as "usual and customary.” Your insurance company typically will reimburse you a percentage of those fees. Before any treatment or procedure, it is a good idea to read your policy or check with your insurance company representative concerning your coverage. We encourage you to bring in your benefit booklet to the office so we can review your coverage with you. Our staff will be happy to assist you in preparing any necessary forms.
Please remember you are fully responsible for all fees charged by this office, regardless of your insurance coverage. Delay in payment increases clerical and administrative costs forcing us to reflect these increases in our fees. We will send you a monthly statement and most insurance companies will respond within four to six weeks. Please call our office if your statement does not reflect your insurance payment within that time frame. Any remaining balance after your insurance has paid is your responsibility. Your prompt remittance is appreciated.
Please bring both your dental and medical insurance information with you to your first visit so that we can expedite reimbursement.

This law ensures continuity of healthcare coverage for individuals changing jobs; includes a provision that impacts on the management of health information; seeks to simplify the administration of health insurance; and aims to combat waste, fraud, and abuse in health insurance and health care.
As HIPAA applies to every patient we see, we will ask you to read, sign and date the HIPAA Privacy Policy Form. Please download the file from this page and bring it to your appointment. We are also happy to supply you with one at the time of your office visit.
